Please answer before every appointment. Your answers help Jai decide whether the selected eye-area service can proceed safely. If you answer Yes to any question, explain below. A current eye infection, significant irritation, or a concerning reaction may mean the service needs to be postponed. Contact your healthcare professional for medical advice.

1. Have you ever had an allergic reaction or significant irritation from lash extensions, adhesive, tape, cleanser, remover or another eye-area product?

2. Do you currently have an eye infection, redness, swelling, discharge, painful or irritated eyelids, or any other active eye concern?

3. Have you had recent eye surgery, laser treatment, injections or a medical eye treatment, or been advised to avoid products or procedures near your eyes?

4. If you answered Yes above, describe the condition, reaction, date and any product or medical advice involved. If all answers are No, enter None.

All four answers are required during booking.

Service consent & risk acknowledgement

I voluntarily request and authorize Jai to perform the selected lash service after reviewing these terms. Products used near the eyes, including adhesive, cleanser and remover, may cause irritation or an allergic reaction, even without a prior reaction. I acknowledge and accept these described risks. I will disclose known allergies, sensitivities, past reactions, current eye concerns and recent eye procedures in the intake questions. Results and wear time vary. I will tell Jai if I experience discomfort; I may stop the service at any time.

Reaction & inherent-risk release

I understand that lash services and products used near the eyes may cause allergic reactions, irritation, redness, swelling or other adverse responses, including delayed reactions, even if I have had lash services before and even when reasonable care is used. I accept these inherent risks. To the extent permitted by law, I release Blink Theory by Jai and Jai from liability for an allergic reaction or other adverse response arising from these inherent risks when the selected service is performed with reasonable care. This release does not cover a failure to follow basic safety practices or any liability that cannot legally be waived.

Arrival, cancellation, rescheduling & no-show

To cancel or reschedule, contact Jai at 662.385.6300 or perfectwink@blinktheorybyjai.com at least 24 hours before your appointment.

• 24 hours or more: No cancellation or rescheduling fee. A deposit may be transferred to an available appointment; cancellation refunds or transfers are at Jai’s discretion.
• Cancellation under 24 hours: 50% of the booked service price.
• Rescheduling under 24 hours: $30, payable before a new appointment is confirmed.
• More than 15 minutes late: Jai may cancel or reschedule if there is insufficient time. If canceled because of late arrival, the fee is 50% of the booked service price.
• No-show: 100% of the booked service price when you do not arrive or contact Jai before your scheduled time.

Amounts already paid are credited toward the applicable fee; any remaining fee is due separately from the service balance. Contact Jai as soon as possible about an emergency. Exceptions are considered individually.

Booking payments

A $30 deposit is due for a full set or fill. Fresh Start Removal requires a $10 deposit. Lash Bath costs $10 total, paid in full at booking. Payments are applied toward the service total; any remaining service balance is due at your appointment.

Fill policy

Blink 1-2 Week Fill appointments are 1 hour: Classic $65, Hybrid $75, Volume $85, and Wispy Signature $95. Blink 3 Week Fill appointments are 1.5 hours: Classic $80, Hybrid $90, Volume $100, and Wispy Signature $110. Fills refresh existing extensions. Jai will assess whether a fill is suitable. If a full set or removal is needed instead, Jai will discuss the service, time and price with me before any change. A different appointment may be needed.

Photo ID

A clear photo of your ID is required in Acuity to secure your appointment. The name on your ID must match your booking name. Contact Jai with questions about ID handling before uploading.

Aftercare

I will follow the aftercare guidance given at my appointment, keep my lashes clean, avoid pulling on extensions and contact Jai if I experience discomfort. I understand that care affects retention. Photo/video permission is asked separately and may be declined without affecting my service.

Required acknowledgements during booking

• I have read and agree to the terms above.
• I have disclosed known allergies or past reactions to lash adhesive, cleansers, removers, latex, tape or eye products, and I understand irritation or allergy can occur.
• I have read the aftercare guidance above and agree to follow Jai’s instructions, keep my lashes clean, avoid pulling extensions, and report discomfort.
• I agree to the cancellation, rescheduling, late-arrival and no-show policies above, including the 24-hour notice requirement, 50% cancellation or canceled late-arrival fee, $30 late-rescheduling fee, and 100% no-show fee. Amounts already paid are credited toward the applicable fee.
• I understand the style-specific fill prices and the 1–2 week and 3 week maintenance timeframes. A fill requires suitable existing extensions; Jai will discuss any needed full set or removal and its price before changing my service.
• I have read the Reaction & Inherent-Risk Release above. I understand that a reaction can occur despite reasonable care, and I accept and agree to the stated release to the extent permitted by law.

Your choice is separate from service consent. Declining permission will not affect your appointment. If you say Yes, Jai may capture lash/eye-area photos or videos and use them in Blink Theory by Jai’s website, portfolio and social media marketing. No full-face image will be used without separate permission. You can contact perfectwink@blinktheorybyjai.com to withdraw permission for future use.
